Notice of Privacy Practices

Note: This is a summary of our HIPAA Notice of Privacy Practices. A complete copy of our Notice of Privacy Practices is provided to all clients during intake and is available upon request.
Your Rights
As a client, you have the right to:
- Request a copy of your health records
- Request corrections to your health information
- Request restrictions on certain uses and disclosures
- Request confidential communications
- Receive an accounting of disclosures
- File a complaint if you believe your privacy rights have been violated
Our Responsibilities
We are required to:
- Maintain the privacy of your protected health information (PHI)
- Provide you with notice of our legal duties and privacy practices
- Notify you if a breach of your unsecured PHI occurs
- Follow the terms of this notice currently in effect
Uses and Disclosures of Your Information
We may use and disclose your health information for treatment, payment, and healthcare operations without your written authorization. Other uses and disclosures require your written authorization, except in specific circumstances outlined in our full Notice (such as emergencies, court orders, or mandatory reporting requirements).
Psychotherapy Notes
Psychotherapy notes receive additional protection under HIPAA. We will not disclose psychotherapy notes without your written authorization, except in limited circumstances as permitted by law.
Minors
Parents/guardians of minor children generally have access to their child's health records. However, Minnesota law provides certain privacy protections for minors in specific circumstances. We will discuss confidentiality expectations at the start of treatment.
